Si vous habitez à l'étranger, ce formulaire est à compléter, signer et envoyer au service compétent selon votre lieu de résidence, accompagné de plusieurs pièces justificatives.

 Attention :

le formulaire cerfa n°14881*01 est à utiliser uniquement si vous habitez à l'étranger. Si vous habitez en France, vous devez faire la demande de permis international via un téléservice disponible sur le site de l'ANTS.
